The following forms may be placed in a separate envelope should you have concerns about others possibly viewing this information returned with other material: All new house staff must call Health Services within four days of receiving the email notice to obtain integral information for employment and schedule an appointment. Your appointment scheduled with Health Services will last approximately 30 minutes. You may make this appointment before orientation begins or upon your move to the Danville area. Limited times will be available during orientation week. Phone: 1-570-214-9424 Toll free: 1-800-275-6401, say Health Services. You will review your records and receive a TB test.
