Employment Information

The following information concerns your employment with Geisinger Health System. There is no paperwork to complete or submit. Please review these documents fully before beginning your orientation at Geisinger:

• W-4 Form Employee’s Withholding Allowance Certificate (.pdf) For review only. You will fill this out in Lawson and submit online during orientation.
• I-9 Employment Eligibility Verification (.pdf) For review only. This form will be completed during orientation. The second page indicates identification document(s) presented during orientation.
• Sample contract with signature page (.pdf) For review only. This form is in your mailed contract. Use as a guide only.
• Sample Pennsylvania MD Training License Request (.pdf) For review only. This form is in your mailed contract. Use as a guide only.
• Sample Pennsylvania DO Training License Request (.pdf) For review only. This form is in your mailed contract. Use as a guide only.

• 2010 Benefit Guide (.pdf) Benefits sign-up is during orientation. Questions will be answered at that time.

• CSERDR (.pdf) Criteria for selection, evaluation, reappointment and dismissal for residents
• Code of Conduct (.pdf) For review only. This will be electronically signed during orientation.

• Direct Deposit Authorization (.pdf) For review only, may be submitted online at orientation or anytime after employment via Lawson.
