Curriculum and rotations

The PGY-1 year is devoted to establishing a solid foundation of knowledge and competency in managing psychiatric patients. Rotations are designed to foster basic abilities in assessment of risk and safety, interviewing and communication regarding cases, construction of a differential diagnosis, implementation of basic treatment planning for the major psychiatric and medical disorders, establishment of a therapeutic alliance, use of the medical literature to research clinical questions, teaching and mentoring medical students, and participation in a multi-disciplinary care team. Our inpatient psychiatry rotations alternate between our two major teaching units, to expose residents to a larger faculty from which to learn; they are four continuous months in length to insure residents are fully incorporated into the daily life of the units.

The PGY-2 year is focused on expanding psychiatric inpatient and consultation evaluative skills, building on the basic foundation. The second-year resident is expected to progressively advance skills in interviewing and examination, generation of a formulation regarding a patient case, documentation and communication of clinical reasoning, management of acute distress, recognition and treatment of intoxication and withdrawal syndromes, independent diagnosis and initial management of major psychiatric presentations on inpatient units, evolution of leadership skills within the care team, selection and initiation of basic psychotherapy, beginning supervision skills of junior team members including delegation and accountability for team tasks, application of the medical literature to ongoing patient cases, and participation in quality and process improvement.

The PGY-3 year expands the resident's skills from inpatient services to outpatient clinics. Building on exposure in the second year to introductory principles of psychotherapy, the third year resident will participate longitudinally in continuity clinics encompassing various settings, including individual psychotherapy, medication management, child and adolescent psychiatry, community psychiatry, and specialty clinics such as intensive outpatient treatment, women's health, and medication-assisted treatment for substance use disorders. Residents will continue weekly individual psychotherapy supervision sessions with senior faculty, branching out into other psychotherapy modalities. Residents in their third year will enjoy a dedicated half-day each week for research endeavors and other academic projects. PGY-3 skills evolve to include teaching and modeling advanced approaches to assessment, mastery of psychopharmacology, developed skill in cognitive and psychodynamic therapies, assimilation of medical evidence into patient care activities, and leadership in quality improvement and patient safety projects within the clinic.

The PGY-4 year focuses on transition to practice, and readies the resident for launch into fellowship training or independent career, from a comprehensive basis as a diagnostician, therapist, and psychopharmacologist. Fourth year residents take on leadership roles within the program and in the setting of patient care. Senior residents will continue to expand their psychotherapy repertoire to include couples, family, and group psychotherapies. Rotations will be comprised of exposure to administrative psychiatry, geriatric and forensic specialties, and state hospital practice. PGY-4 residents will return to the inpatient and consultation services as senior resident instructors/junior attendings leading care teams, with emphasis on mentorship and teaching for more junior learners. Fourth-year residents will enjoy elective time to finely tailor their skills in the areas of their choice, and they will benefit from a dedicated research month to complete and present their academic research project.
