About us
Geisinger’s Vascular Neurology Fellowship program has a simple goal — to train well rounded, dynamic leaders in a specialty that requires calm, capable thinkers who are willing to stay current with research and innovation in the field.
Our 1-year, ACGME-accredited Vascular Neurology Fellowship is headquartered at Geisinger Medical Center in Danville, Pa. We have a Joint Commission-certified Comprehensive Stroke Center with a large referral base, and we’re part of a system with 6 Comprehensive and Primary Stroke Centers that serve rural and urban areas. Program overview
As a Vascular Neurology fellow, you’ll leave our program ready to:
- Provide high-quality clinical care and assessment of patients with various cerebrovascular disorders in different settings (inpatient, telemedicine and outpatient), including diagnostic evaluation, treatment, management, counseling and prevention
- Manage critically ill neurological patients, such as those with cerebral hemorrhage, brain aneurysms or vascular malformations
- Work effectively with multidisciplinary teams
- Develop competency in the interpretation and application of evidence-based medicine
- Understand the role of quality improvement and policy-building in optimizing care for our vascular neurology patients
Our fellowship prepares you for American Board of Psychiatry and Neurology (ABPN) vascular neurology board certification and a successful career as either an academic vascular neurologist or neurohospitalist.
Your clinical year will provide you with the rotations in vascular neurology you’ll need for board eligibility. You’ll gain extensive experience in the management of acute ischemic stroke with intravenous thrombolytics and endovascular methods, including intra-arterial thrombolysis and thrombectomy. And you’ll become comfortable interpreting neuroimaging including MRI, CT angiography and conventional angiography.
Acute inpatient stroke: This rotation will refine your knowledge of state-of-the-art acute stroke management. Fellows will develop leadership and educational skills by working closely with board-certified vascular neurology attendings, so you’re ready to lead a team of stroke advanced practitioners.
You’ll participate in daily rounds on stroke patients, including in the ICU. Our stroke service sees patients in the Neuro ICU in conjunction with Critical Care Medicine. Fellows serve as an adjunct to the attending with responsibilities delegated as the attending sees fit, with gradual progression to greater autonomy.
Endovascular neurosurgery/ICU: Fellows participate in the management of robust clinical cases of the Geisinger Medical Center Neuroendovascular group, which is led by 3 endovascular neurosurgeons, residents, advanced registered nurse practitioners and an endovascular fellow. Cases include mechanical thrombectomy for acute ischemic stroke, repair of cerebral aneurysm and vascular malformations, and carotid endarterectomy and stenting.
Teleneurology: Geisinger has a robust telehealth network serving more than 12 hospitals. As a hub for tertiary stroke care, you will learn to triage and assess vascular neurology cases using state-of-the-art video technologies. Fellows work in a busy teleneurology service in inpatient and outpatient clinical settings.
Neurovascular continuity clinic: You’ll refine your longitudinal management of stroke patients by following select patients after inpatient discharge in a dedicated fellow continuity clinic. The clinic will incorporate telehealth in seeing patients across various outpatient settings, from the clinic to the home.
Research: We provide opportunities ranging from clinical-based research to quality improvement. You’ll be expected to participate in at least 1 project with the goal of garnering a publication or poster at an international conference.
Our program cultivates fellows’ personal interests to give them the exposure and experience needed to build toward their long-term career goals. Future electives can be added and developed based on fellows’ feedback.
Neurocritical care: Fellows spend at least 4 weeks in the Neuro ICU to gain greater aptitude in caring for critically ill vascular neurology patients.
Neuroradiology: Fellows spend at least 2 weeks working with our neuroradiologists to gain proficiency in reading and interpreting neuroimaging studies.
Neuroendovascular: Fellows spend at least 2 weeks on rotation with our neurointerventionalists to participate in complex endovascular cases.
Inpatient telemedicine: Fellows spend at least 2 weeks working with our inpatient teleneurologists, learning to manage and triage neurologic emergencies and consultations across the system.
Research: Fellows spend at least 4 weeks conducting research and fulfilling academic pursuits. Faculty will be on hand to help develop and collaborate on projects as needed.
Neurosonology: Fellows interested in learning neurosonology can read with our certified providers. This rotation will be valuable for those fellows seeking certification in interpreting transcranial doppler studies.
