• Medical school transcript
• USMLE transcript/COMLEX transcript (passage on first attempt)
• Valid ECFMG certificate if an international medical graduate
• Three recent clinical letters of recommendation
• Personal statement explaining your interest in our program and in family medicine
• Medical school graduation date
