Alien information request form

Non-U.S. citizens are required to complete the Alien Information Request Form in your onboarding package. This form allows Geisinger to collect information for tax withholding purposes regarding immigration category and the primary reason the person is in the U.S. This form and all supporting documents must be submitted before you receive your first paycheck.
