Sarah F. Hale, PharmD, BCPPS
Director of Pharmacy, Geisinger Medical Center
Director, Health System Pharmacy Administration and Leadership Residency
Dr. Hale earned her Doctor of Pharmacy from Duquesne University’s Mylan School of Pharmacy. She joined Geisinger Medical Center in 2007 as a clinical pharmacist in pediatrics. She completed a PGY1 pharmacy practice residency at Albany Medical Center in 2012, followed by a PGY2 residency in critical care and pediatrics in 2013. She is a board-certified pediatric pharmacist and completed a scholarship of teaching and learning program through Albany College of Pharmacy and Health Sciences. She is presently the Director of Pharmacy for Geisinger Medical Center, where she also serves as the director of the PGY1/PGY2 Health System Pharmacy Administration and Leadership (HSPAL) residency program. Dr. Hale is a member of Geisinger’s Academy of Educators and serves on the Faculty Development Advisory Committee. She is adjunct clinical faculty for the Department of Pharmacy Practice at Wilkes University School of Pharmacy and a graduate of the Partners in Leadership program at Geisinger. She serves as a preceptor for the pharmacy administration learning experiences. Her professional interests include pharmacy leadership, pediatric critical care, medication safety, preceptor and faculty development, and pharmacy education.
Michael A. Evans, RPh, MBA, FASHP
Vice President, Enterprise Pharmacy
Chief Pharmacy Officer
Michael Evans is the vice president of Enterprise Pharmacy and is responsible for overall strategy development, administrative oversight and leadership for pharmacy services at Geisinger. He is a graduate of the Philadelphia College of Pharmacy and received an MBA in Healthcare Management from Western Governors University. He and his pharmacy leadership team have successfully built a robust network of pharmacy programs for Geisinger’s integrated delivery system. The programs consist of Hospital programs, Medication Therapy Disease Management programs, Retail Pharmacies, Mail Order Pharmacy, Specialty Pharmacies, Telepharmacy, Managed Care Pharmacy, Infusion Pharmacy, Formulary Services, Informatics, Center for Pharmacy Innovation and Outcomes (CPIO), Pharmacy Fellowship and Residency programs and Pharmacy Planning and Strategy. All 400+ pharmacists in Enterprise Pharmacy are in a collaborative practice model independent of site of practice, which facilitates moving Geisinger Pharmacy Programs to the forefront of multidisciplinary patient care focusing on quality, satisfaction, access and value.
Bradley Dudeck, PharmD, BCPS
Assistant Director of Pharmacy, Inventory Management
Dr. Dudeck is a graduate of the Nesbitt School of Pharmacy at Wilkes-University. He earned his Doctor of Pharmacy degree in 2013. After graduation, he completed a PGY1 Pharmacy Practice Residency at Moses Taylor Hospital in Scranton, Pa. He also obtained a Teaching and Learning Certificate through Wilkes-University. Prior to coming to Geisinger, Dr. Dudeck served in multiple capacities through his career from being a Clinical Pharmacist, Clinical Coordinator, to a Director of Pharmacy. Dr. Dudeck joined Geisinger in 2022.
Sara Gaines, PharmD, BCPS
Clinical Pharmacist, Hepatology
Director, PGY1 Pharmacy Residency Program
Dr. Gaines is a graduate of Ohio Northern University Raabe College of Pharmacy. She earned her Doctor of Pharmacy in 2008. After graduation, she completed a PGY1 pharmacy practice residency at Conemaugh Memorial Medical Center in Johnstown, PA. Following residency, Dr. Gaines practiced in both an acute care and ambulatory care role. In 2016, she accepted a newly created position in ambulatory care at Geisinger Medical Center in the gastroenterology/hepatology clinic, where she manages patients with hepatologic diseases. She serves on several committees, including the Residency Advisory Committee for both the PGY1 and PGY1/2 HSPAL residencies and the Medication Utilization Evaluation Committee. Dr. Gaines serves as a preceptor for the Orientation, Self-Assessment and Professional Performance Improvement, and Hepatology learning experiences. Her professional interests include viral diseases, solid organ transplant, teaching/precepting, in addition to well-being and resilience.
Stephanie Gill, PharmD
Assistant Director of Pharmacy, Geisinger Medical Center
Dr. Gill is a graduate of University of Pittsburgh School of Pharmacy. She earned her Doctor of Pharmacy degree in 2002. She has practiced as a clinical pharmacist at Geisinger Medical Center since 2003 with experience in several areas of both hospital pharmacy, including nephrology and solid organ transplant, as well as ambulatory pharmacy. In 2019, she accepted a position as the Pharmacy Manager for Geisinger Medical Center, prior to moving into her current role as assistant director. Dr. Gill serves as a preceptor for the Acute Care Operations learning experience in the HSPAL program and the Leadership and Management learning experiences for the PGY1 and PGY2 critical care programs.
Stacey Grassi, PharmD, CACP
Assistant Director, Medication Therapy Disease Management, Family Practice
Dr. Grassi is a graduate of Nesbitt School of Pharmacy at Wilkes University, where she earned her Doctor of Pharmacy degree in 2002. After graduation she completed a PGY-1 Pharmacy Residency at Geisinger Medical Center in 2003. After residency she accepted a position as a clinical pharmacist at Geisinger where she practiced in the areas of general internal medicine, nephrology, hematology and oncology, and anticoagulation. In 2013, she accepted the Regional Coordinator of Ambulatory Clinical Pharmacy Programs position for the Central Region. In 2016, she accepted the position of Assistant Director of Ambulatory Clinical Pharmacy Programs for the Central Region where her oversight included anticoagulation, diabetes, hypertension, lipids, heart failure, asthma, COPD, pain therapy, addiction medicine, hepatitis C, IBD, anemia in CKD and pregnancy, and transplant. In 2021, she accepted the position of Assistant Director of Ambulatory Clinical Pharmacy Programs to have oversight of all Family Practice Clinics within the Geisinger Health System. She also serves as an administrative preceptor for the Geisinger PGY1/PGY2 HSPAL Pharmacy Residency Program, the Geisinger Clinic Northeast PGY1 Residency Program and the Geisinger Clinic Central PGY1 Residency Program. She is an adjunct assistant professor in the Department of Pharmacy Practice at the Nesbitt School of Pharmacy at Wilkes University.
Jerry Greskovic, RPh, MBA
Associate Vice President, Ambulatory Pharmacy Programs
Jerry Greskovic is a graduate of the Philadelphia College of Pharmacy at the University of the Sciences. He earned his Bachelor of Science in Pharmacy in 1991 and an MBA in Healthcare Management from Western Governors University in 2020. He spent 11 years working in retail pharmacy, pharmacy management and hospital pharmacy before accepting a position in 2002 as a clinical pharmacist at Geisinger, specializing in chronic disease management. In his current role as Associate Vice President, Mr. Greskovic is responsible for the oversight of Geisinger’s Retail, Home Infusion, Specialty and Mail Order Pharmacies. In addition, he oversees Geisinger Pharmacy’s Centralized Clinical Pharmacy Services Program, Pharmaceutical Reimbursement and Financial Assistance Department and Geisinger’s Medication Therapy Disease Management (MTDM) Program, which was the recipient of the 2016 American Society of Health-System Pharmacists (ASHP) Award of Excellence and the 2016 Pennsylvania Society of Health-System Pharmacists (PSHP) Innovative and Collaborative Practice Award. Mr. Greskovic has authored multiple publications and has presented at both the state and national level on topics such as Ambulatory Pharmacy Disease Management, Population Health, and Pain Management. Additionally, he is the co-chair of PSHP’s Ambulatory Care Committee, sat on the Philadelphia College of Pharmacy and Science Board of Visitors for 4 years, and in 2020 was honored to receive PSHP’s Pharmacist of the Year Award.
Kelly Guza, PharmD, MBA, BCPS
Associate Vice President, Acute Care Pharmacy Services
Dr. Guza is a graduate of the Philadelphia College of Pharmacy at the University of the Sciences. She earned her Bachelor of Science in Pharmacy in 1998 and Doctor of Pharmacy in 1999. She has practiced as a clinical pharmacist at Geisinger Medical Center since 1999 and has experience in several areas, including internal medicine, nephrology, anticoagulation medication therapy management and pediatrics. Dr. Guza practiced as a pediatric clinical pharmacist, specializing in pediatric intensive care and pediatric hematology and oncology for 10 years prior to taking a management role as assistant director of Geisinger’s Central Region. In her current role, she oversees pharmacy clinical programs for the acute care setting across the health system. Dr. Guza is an educator for the executive pharmacy leadership rotation.
David Klinger, RPh, MHL
System Director, Operations and Compliance
David Klinger is a graduate of Temple University and earned a Master of Healthcare Leadership from Western Governors University. He also completed the American Society of Health-System Pharmacists’ Pharmacy Leadership Academy. Mr. Klinger has been with Geisinger since 2004. During his tenure at Geisinger, he has practiced as a clinical pharmacist in several areas, including internal medicine and hematology/oncology. He also spent five years as director of pharmacy at Geisinger Medical Center and three years as associate vice president for acute care pharmacy. Mr. Klinger has led projects related to the opening of several hospital and surgical center pharmacies. He also led the implementation of automated dispensing cabinets (ADCs) in the operating rooms as well as the systemwide upgrades of existing ADCs. He is responsible for the oversight of Geisinger’s 340B, medication safety, corporate compliance, controlled substance management and automated medication dispensing programs.
Staley Lawes, PharmD, BCPS, FISMP
Assistant Director of Pharmacy, Geisinger Medical Center
Dr. Lawes earned a PharmD from Philadelphia College of Pharmacy in 2010. She accepted a position as a clinical pharmacist at Geisinger Medical Center working in internal medicine from 2010 to 2014. Then she moved to New Jersey, where she worked as a staff pharmacist at CentraState Medical Center. In 2015, she created a pharmacy position in the Emergency Department with the goal of improving admission medication reconciliation. Dr. Lawes completed a fellowship with the Institute for Safe Medication Practices (ISMP) in 2017 and subsequently accepted a position as medication safety officer at JFK Medical Center. In 2022, she rejoined Geisinger Medical Center in Danville as assistant director of pharmacy. Dr. Lawes serves on the newsletter advisory board for ISMP. Her professional interests include medication safety and quality improvement.
Daniel S. Longyhore, PharmD, EdD, BCACP
System Director, Knowledge Management
Dr. Longyhore is a graduate of the Nesbitt School of Pharmacy and the School of Education at Wilkes University. He completed a pharmacy practice residency with Palmetto Health Richland in Columbia, S.C., and family medicine pharmacy residency with St. Louis College of Pharmacy and St. John’s-Mercy Medical Center in St. Louis, Mo. As a system director, Dr. Longyhore oversees pharmacy residency programs for Geisinger and directs the professional development and collaborative practice agreements for pharmacists and pharmacy technicians in the health system. He is an active member of the American College of Clinical Pharmacy and serves as director of their Teaching and Learning Certificate Program Academy.
Dina Sandri, RPh
Clinical Pharmacist and Pharmacy Manager, Geisinger Shamokin Area Community Hospital
Dina Sandri is a graduate of Philadelphia College of Pharmacy and Sciences. She has served as a Clinical Pharmacist for Geisinger since 2012 and has been the Inpatient Pharmacy Manager of the Shamokin Campus since 2016. She serves as a preceptor for the internal medicine rotation and the PGY2 Health- System Pharmacy Administration and Leadership. Additionally, she precepts and mentors pharmacy students, including Clinical Co-ops, IPPE and APPEs. She serves on various committees including MUE Committee, Medication Safety Committee, Formulary Steering Committee, Pharmacy and Therapeutics Committee and many leadership committees. Her areas of interest include precepting, medication safety, anticoagulation and internal medicine.
Leslie Shumlas, PharmD
Pharmacist, Geisinger Health Plan
Dr. Shumlas is a health plan pharmacist at Geisinger Health Plan. She received her Doctor of Pharmacy from Wilkes University in 2019. Dr. Shumlas completed a PGY1 managed care pharmacy residency at Highmark Inc. in Pittsburgh, also earning a teaching and learning certificate through Duquesne University. She has experience in utilization management, evidence-based medicine, medication therapy management, case and disease management, and data analytics.
Stacey Tonitis, PharmD
Assistant Director, Retail and Mail Order Pharmacy
Dr. Tonitis is a graduate of the Nesbitt School of Pharmacy at Wilkes University, where she earned her Doctor of Pharmacy in 2008. She began her career at Geisinger in 2022 as assistant director of retail and mail order pharmacy. Prior to joining Geisinger, she held various leadership roles within retail pharmacy. She is responsible for the oversight of Geisinger outpatient pharmacies and mail order facility. Dr. Tonitis’ work is focused on improving access and ease of use for patients through interprofessional collaboration. In the past, she has served as a preceptor in pharmacy business leadership. She currently serves as preceptor for HSPAL residency program. She is also serving as a preceptor in a resident project centered on bedside medication delivery.
Leeann Webster, RPh, MBA
Director, Medication Therapy Disease Management
Leeann Webster is a graduate of the University of Pittsburgh School of Pharmacy and a 2019 graduate of The University of Scranton Kania School of Management. She is also a certified diabetes care and education specialist. She practiced as a clinical pharmacist in Geisinger’s Medication Therapy Disease Management (MTDM) Clinics for 15 years before moving into an administrative role within the program for the past six years. Ms. Webster’s responsibilities include oversight of Geisinger’s MTDM programs and Telepharmacy Refill Call Center. Geisinger’s MTDM Program includes 77 clinical pharmacists embedded within 67 primary care and specialty clinics who manage chronic disease under a collaborative practice agreement with the providers. The Telepharmacy Refill Call Center includes 31 pharmacists and 61 technicians who manage all refill requests and medication-related electronic communication within the health system — approximately 89,000 encounters per month. Geisinger’s clinical pharmacist collaborative practice model provides patients with acute and chronic disease management of multiple chronic disease states including, but not limited to, anticoagulation, diabetes, heart failure, COPD, pain management, neurology/MS, oral chemotherapy, rheumatology, hepatitis C, transplant, endocrinology, dermatology and the home-based medical care program Geisinger at Home. The MTDM program was named the recipient of the American Society of Health-System Pharmacists (ASHP) Award of Excellence for 2016 and a finalist in medication safety for chronic pain management.
Durga Zally, PharmD
Director, Hematology-Oncology Pharmacy Services
Dr. Zally is a graduate of Midwestern University School of Pharmacy. Prior to receiving her Doctor of Pharmacy, she earned a Bachelor of Science in Biotechnology from William Paterson University and was employed as an analytical R&D scientist at Hoffman-La Roche. Prior to her career at Geisinger, Dr. Zally practiced as a clinical pharmacist at HealthSouth Rehabilitation Hospital as well as Mount Nittany Medical Center in Pennsylvania. She was also previously a pharmacy manager as well as a staff pharmacist at Target Pharmacy. Dr. Zally joined Geisinger as a hematology/oncology clinical pharmacist in 2014 at Geisinger Windmere and Scenery Park, advancing to the role of assistant director for hematology/oncology in 2018. She continues to redesign and advance pharmacy’s role in hematology/oncology at Geisinger through her role as systemwide director of hematology/oncology pharmacy.
Adriene Zook, PharmD
Pharmacy Manager, Ambulatory Clinical Pharmacy Programs – Family Practice
Dr. Zook is a graduate of Philadelphia College of Pharmacy, where she earned her Doctor of Pharmacy in 2011. Upon graduation, Dr. Zook continued her time transitioning from a graduate extern to a licensed clinical pharmacist, ultimately landing in Geisinger Montoursville Family Practice, where she began the MTDM services there. Her practice grew and she remained in this role for nearly nine years until taking the position of regional manager for ambulatory programs for Geisinger’s Central Region. After a January 2022 leadership restructure, Dr. Zook remains a pharmacy manager but has oversight of the entire Family Practice Ambulatory Pharmacy Program, assisting the clinical pharmacists embedded at the primary care locations with program growth and development, day-to-day workflow asks and requirements, and quality and data tracking tasks. Through her role as a preceptor for the administration rotation, Dr. Zook can continue to capture her passion for education not only for her providers and pharmacists, but the pharmacy residents as well.
