Sarah Krahe Dombrowski, PharmD, BCACP
PGY1 Residency Program Director
Clinical Pharmacist, Ambulatory Clinical Pharmacy Programs
Dr. Dombrowski is a graduate of the University of Pittsburgh School of Pharmacy, where she received her Doctor of Pharmacy in 2012. She has an undergraduate degree in biochemistry and molecular biology from Penn State University and is excited to be back in Happy Valley. After pharmacy school, Dr. Dombrowski completed a PGY1 community pharmacy residency with the University of Pittsburgh and Rite Aid and a PGY2 specialty residency in ambulatory care/family medicine with UPMC St. Margaret. Following residency, she taught at LECOM School of Pharmacy before accepting a position with Geisinger as an ambulatory clinical pharmacist. She is the director of the residency and a preceptor for the geriatrics learning experience at Geisinger 65 Forward, as well as the orientation, research, and leadership and teaching experiences. Her professional interests include management of chronic disease, pharmacy education and interprofessional collaboration.
Elena Marines, PharmD

Dr. Marines graduated from Wilkes University’s Nesbitt School of Pharmacy in May 2020. She completed her PGY1 pharmacy residency with Geisinger Clinics West. After graduation from her residency, she accepted a position as an MTDM clinical pharmacist in the Geisinger Lewistown and Reedsville clinics before moving to Geisinger Bellefonte. Her professional interests include chronic disease state management, with a special interest in geriatrics.
Allison Cebulko, PharmD, BCCCP
Clinical Pharmacist, Neuroscience Services
Dr. Cebulko is a graduate of the Nesbitt School of Pharmacy at Wilkes University, where she earned her Doctor of Pharmacy in 2014. She then completed a PGY1 pharmacy practice residency through WellSpan York Hospital. During that year, she obtained her pharmacy teaching certificate through Shenandoah University. After graduation, she accepted an inpatient position at Geisinger Medical Center. In 2016, she transferred to Geisinger Community Medical Center, where she focused in emergency medicine and critical care. She recently transitioned to the ambulatory care setting working in the Neurology Department. Together, the neurology pharmacists co-manage disease-modifying therapies in multiple sclerosis, immune modulation therapy in the neuroimmune subspecialty, antiepileptic dosing in pregnancy and antiplatelet dosing in patients undergoing neurovascular intervention. Dr. Cebulko serves as a preceptor for the pharmacogenomics learning experience.
Bernard Eck, BS Pharmacy, CACP, CDE

Bernie Eck is a graduate of Temple University School of Pharmacy in 1986. While a student, he participated in nuclear pharmacy research and worked at a nuclear pharmacy. After graduation, he worked in community pharmacy for two years. He worked as a pharmacist at Lewistown Hospital for three years and was director of pharmacy at HealthSouth Nittany Valley Rehabilitation Hospital. As the director, he initiated clinical programs, aminoglycoside monitoring and anticoagulation monitoring. He then accepted a position as an ambulatory clinical pharmacist with Geisinger, specializing in anticoagulation initially, then expanding to asthma, diabetes, dyslipidemia and hypertension management plus benzodiazepine tapering. His professional interests include patient education overall, but particularly diabetes education. Mr. Eck is a preceptor for the anticoagulation rotation and primary care learning experience at Geisinger Scenery Park.
Christopher Evans, PharmD
Clinical Pharmacist, Ambulatory Clinical Pharmacy Programs
Dr. Evans is a graduate of Lake Erie College of Osteopathic Medicine – Erie School of Pharmacy in 2020. After graduation he went on to complete his general PGY1 residency at Conemaugh Memorial Medical Center. Upon completion of his PGY1 he accepted a position as an ambulatory clinical pharmacist with Geisinger, specializing in management of asthma, COPD, diabetes, dyslipidemia, hypertension, anticoagulation and chronic pain. He is a published author, with research in the field of pharmacy interventions and A1c outcomes in diabetes. Dr. Evans is a preceptor for the anticoagulation, primary care and on-call learning experiences at Geisinger Scenery Park and is involved in longitudinal topic discussion for the residency. His professional interests include management of diabetes, anticoagulation and chronic pain.
Alison F. Flango, PharmD, CACP, CDE

Dr. Flango is a graduate of the Bernard J. Dunn School of Pharmacy at Shenandoah University. She earned her Doctor of Pharmacy in 2005. After graduation, she worked for CVS Pharmacy in Lewistown, Pa. She accepted a position as a clinical pharmacist with Geisinger, specializing in anticoagulation in 2010. She has since completed the Geisinger competencies for diabetes, dyslipidemia, asthma/COPD, heart failure and hypertension management and is practicing as an ambulatory pharmacist. Dr. Flango is a preceptor for the management and administration learning experience. Her professional interests include diabetes, patient education and teaching. In her current role, she manages the daily operations of 15 MTDM clinics and 18 clinical pharmacists.
Samantha Kitko, PharmD

Dr. Kitko graduated from Lake Erie College of Osteopathic Medicine School of Pharmacy in May 2021. Upon graduation, she completed her PGY1 pharmacy residency with Geisinger Clinic West. She then accepted a position as an MTDM clinical pharmacist in the Geisinger Bellefonte clinic and is now practicing at the Moshannon Valley clinic. Her professional interests include chronic disease state management, including diabetes, anticoagulation, smoking cessation, and she has a special interest in geriatric care. Dr. Kitko is a wellness mentor and coordinator for the residents.

Dr. Kline is a graduate of the Nesbitt School of Pharmacy at Wilkes University. She earned her Doctor of Pharmacy in 2015. After graduation she accepted a position as an ambulatory clinical pharmacist with Geisinger, specializing in anticoagulation, diabetes, dyslipidemia, asthma, COPD and hypertension management. Dr. Kline is a preceptor for wellness activities and the medication-use evaluation (MUE) projects. Her professional interests include anticoagulation and chronic disease management.
Alysha Lopez, PharmD

Dr. Lopez is a graduate of Wilkes University Nesbitt School of Pharmacy, where she received her Doctor of Pharmacy in 2015. She also has an honorary undergraduate degree in biological sciences from Wilkes University after completing coursework from both Wilkes and the University of Pittsburgh. Originally from DuBois, Pa., Dr. Lopez resides in Belleville, Pa. After pharmacy school, she worked as a clinical pharmacist at Geisinger Lewistown Hospital, then transitioned to Geisinger Windmere Pharmacy, practicing primarily in hematology and oncology. Dr. Lopez has her certificate in compounded sterile preparations from the American Society of Health-System Pharmacists (ASHP) and is working toward being oncology certified by the American College of Clinical Pharmacy (ACCP). She is the coordinator for hematology, oncology and IV sterile compounding for the Western Region of Acute Care Pharmacy. During her career at Geisinger, Dr. Lopez has served on multiple committees that function to better implement patient care, such as Hem/Onc BEACON, Hem/Onc Education and Knowledge Management, Lewistown LEAN and P&T Order Set. She has a passion for streamlined and direct patient care and helps to spearhead and operationalize patient transitions from clinic infusions to getting infusions in their own homes. Dr. Lopez also enjoys her time with other pharmacists serving on the Pennsylvania Pharmacists Association (PPA) Health System Advocacy Committee and Awards Committee. Her professional interests include medical oncology infusions and oral chemotherapy, pain management, palliative care, diabetes management, sterile IV compounding and pharmacy regulations.
Erica Madden, PharmD
Dr. Madden is a graduate of Wilkes University Nesbitt School of Pharmacy/Nursing and received her Doctor of Pharmacy in 2010. After graduation she worked as a retail pharmacist with CVS for 13 years. She transitioned to the Geisinger centralized clinical pharmacy services (CCPS) department in January 2020. Her interests include chronic disease management, pharmacy education, patient education and interprofessional collaboration. While at CCPS, she has assisted with UTI management, medication prior authorization review/submission, transition of the Carbondale clinic into our workflow, development of our departmental training guide and development of smart-phrases and workflow processes as well as contributed as a peer-to-peer review moderator. Most recently she has been a part of the non-CE diabetes initiative for the Geisinger Health Plan. She has a passion for diabetes education/management. Dr. Madden is a preceptor for the staffing learning experience.
Frank Mellace, BS Pharm
Pharmacy Manager, Geisinger Pharmacy Grays Woods
Frank Mellace earned a Bachelor of Science in Exercise Physiology from The Pennsylvania State University (1987), and a Bachelor of Science in Pharmacy from the University of Wisconsin – Madison (1992). Mr. Mellace has worked as a pharmacist in the State College area since 1994 and has extensive community pharmacy management experience. Mr. Mellace has worked at Geisinger for 16 years and manages the outpatient pharmacy at Geisinger Healthplex State College. He is primarily responsible for inventory management, maintaining compliance with state and federal pharmacy guidelines, patient counseling, and hiring, training and supervising a team of pharmacy technicians. Mr. Mellace has been instrumental in developing and implementing the Meds to Beds Program for the outpatient surgery center at Healthplex State College. He is responsible for expanding these bedside medication services to the Mount Nittany Medical Center to help accommodate their discharge patients. He also developed and implemented the blister packing program at Healthplex State College to better serve the medication needs of the Geisinger at Home patients. Mr. Mellace participated in the development and implementation of the recently instituted USP-800 policy and the vaccine policy across all Geisinger Pharmacy locations.
Colleen Strouse, PharmD, BCACP

Dr. Strouse is a graduate of Lake Erie College of Osteopathic Medicine. She earned her Bachelor of Science in Biology in 2011 and her Doctor of Pharmacy in 2015. After graduation, she completed a PGY1 pharmacy practice residency at Niagara Falls Memorial Medical Center in Niagara Falls, N.Y. Following residency, she accepted a position as an ambulatory clinical pharmacist with Geisinger, specializing in anticoagulation, asthma, diabetes, dyslipidemia, hypertension and pain management. Her professional interests include diabetes, cardiology and pain management. She is the preceptor for the pain management learning experience at Geisinger Healthplex State College.
Julia Swigart, PharmD, BCACP, CACP

Dr. Swigart is a graduate of the Nesbitt School of Pharmacy at Wilkes University, earning her Doctor of Pharmacy in 2007. After graduation, she accepted a position at Geisinger Medical Center in Danville, Pa., as an inpatient pharmacist. In 2010, she transitioned into her current ambulatory care position at the Geisinger Lewisburg clinic. Dr. Swigart is a preceptor for the drug information learning experience. Her professional interests include anticoagulation and chronic disease management.
Ryley Uber, PharmD
Clinical Pharmacist, Pharmacogenomics
Dr. Uber graduated from Cedarville University School of Pharmacy with his Doctor of Pharmacy in 2018. Upon graduation, he began a two-year fellowship in clinical pharmacogenomics at the University of Pittsburgh School of Pharmacy. During fellowship, he ran the pharmacogenomics testing service for the cardiac catheterization laboratory and conducted clinical research, including clinical service outcomes and machine learning development. Prior to fellowship completion, he accepted his current position at Geisinger in 2020. In this position, Dr. Uber’s time is split between pharmacogenomics research and implementation throughout the health system. Dr. Uber is a preceptor for the pharmacogenomics learning experience.
Theron Ward, PharmD, BCACP

Dr. Ward is a graduate of the Nesbitt School of Pharmacy at Wilkes University. He earned his Doctor of Pharmacy in 2013. After graduation he accepted a position as an ambulatory clinical pharmacist with Geisinger, specializing in anticoagulation, diabetes, dyslipidemia, asthma, hypertension and heart failure management. Dr. Ward is a preceptor for drug information and administration and management learning experiences. His professional interests include diabetes, heart failure and information mastery.
Keturah C. Weaver, PharmD, BCPS

Dr. Weaver is a graduate of Lake Erie College of Osteopathic Medicine School of Pharmacy. She completed her pre-pharmacy studies at the University of Pittsburgh and went on to earn her Doctor of Pharmacy in 2018. After graduation, she completed a PGY1 pharmacy practice residency at Conemaugh Memorial Medical Center in Johnstown, Pa. She accepted a position as a cardiology clinical pharmacist with Geisinger after graduating her residency. Dr. Weaver is a preceptor for the cardiology learning experience as well as residency program director for the Geisinger Lewistown Hospital PGY1 Residency program. Her professional interests include diabetes, hypertension, heart failure, dyslipidemia, smoking cessation, teaching and transitions of care.
Adriene Zook, PharmD
Pharmacy Manager, Ambulatory Clinical Pharmacy Programs
Dr. Zook is a graduate of Philadelphia College of Pharmacy, where she earned her Doctor of Pharmacy in 2011. Upon graduation Dr. Zook continued her time transitioning from a graduate extern to a licensed clinical pharmacist, ultimately landing in Geisinger Montoursville Family Practice, where she began the MTDM services there. Her practice grew and she remained in this role for nearly nine years until taking the position of regional manager for ambulatory programs for the Geisinger central region. After a leadership restructure in January 2022, Dr. Zook remains in the pharmacy manager role but has oversight over the entire Family Practice Ambulatory Pharmacy Program and assists the clinical pharmacists embedded at the primary care locations with program growth and development, day-to-day workflow asks and requirements, and quality and data-tracking tasks. Through her role as a preceptor for the administration rotation, Dr. Zook can continue to capture her passion for education, not only for her providers and pharmacists, but for the pharmacy residents too.
