Program overview

Our program is varied and gives you the hands-on training you need to thrive. 

You’ll do one-month rotations, with assignments to:

  • GI procedures – Experience with outpatient and inpatient procedures in tertiary care hospital center as well as outpatient surgical center
  • GI consults – Inpatient consult service
  • Hepatology consults – Inpatient consult service managing patients with decompensated cirrhosis as well as pre- and post-liver transplant patients
  • Motility/manometry/procedures – Outpatient focused rotation with exposure to high resolution manometry, anorectal manometry, endoflip, POEM procedures
  • Nutrition – Rotate with the multidisciplinary inpatient team managing nutrition support patient as well as bariatric surgery patients | outpatient rotation with high-risk obesity patients as well as peg tube clinic.
  • Geisinger Shamokin/GI Lab – Community-based hospital rotation comprising inpatient consults, inpatient and outpatient procedures and clinic | GI Lab rotation is coupled with this where fellows get experience in reading BRAVO, capsule endoscopy & breath tests
  • Elective options:
    • GI/radiology – Rotating with body image radiology residents and attendings to get comfortable with reading CTs and MRIs
    • Global health – Travel with our APD, Dr. Addissie, to Ethiopia to learn with GI fellows and attendings to see unique pathology and management of GI diseases in low resource settings
    • Intensive Outpatient IBD – Rotate with Allegheny General Hospital for an outpatient emersion into IBD management with a robust multidisciplinary clinic
    • GI motility – Spend time emersed in evaluating and managing patients with primary motility issues, understanding how to perform, read and utilize tests such as high-resolution manometry, anorectal manometry, smart pill and sitz marker
    • Colorectal surgery – Rotate with our Colorectal surgeons to get some hands-on experience with hemorrhoid therapy, colonic inertia, pelvic floor dysfunction and surgical management of IBD
    • Minimally invasive surgery – Rotate with our minimally invasive surgeons to learn more about bariatric surgery, surgical management of GERD and hiatal hernia. We are a center of excellence for Bariatric Surgery, performing over 50 bariatric surgeries a month.
    • Research/procedure – Rotation that can be modified for research only, procedures only or a hybrid of both
    • Additional elective available upon request and availability

Clinic:

Fellows have continuity hepatology and GI clinic varying from one half day to three half days per week on every rotation except for consults. They see anywhere from three to six patients during these half days and staff patients with the supervising physician. While on nutrition, they have PEG-tube and malnutrition clinic. While at Geisinger Shamokin the fellows have one half day of clinic evaluating patients in a community setting. They also have the opportunity to experience the bariatric and weight loss clinic, transplant clinic, lynch clinic, high risk pancreas clinic and advanced biliary clinic if they have an interest; however, these clinics aren’t required.

Didactics:

Protected time for didactics is Wednesdays from 7 to 8 a.m., 12:30 to 1:30 p.m. and 4 to 5 p.m. Fellows don’t have patients scheduled during this time. Topics include core curriculum lectures by staff and guest speakers on GI-related topics; Case Management, which is an interactive walk through a case presented by a fellow while discussing relevant GI differentials, clinical presentations and management; State of the Art, which is an expert presentation on a specific topic; advanced endoscopy conference, which is a case presentation highlighting a complex advanced GI case; Journal Club; Morbidity & Mortality and board review with fellows as well as faculty. There are also multiple multidisciplinary conferences including IBD Conference, GI Radiology Conference, Liver Histopathology, Luminal Histopathology, Pelvic Floor MDC, Liver Tumor Board, Liver Transplant Committee, GI MDC and Foregut Conference. There is also a monthly multi-institutional conference focused on discussion of challenging and interesting pancreaticobiliary cases called PB Live.

Hands on Labs:

Throughout the year, we have hand-on labs with animal and plastic models to improve old techniques and practice new techniques ranging from clips, snares, injections to savary dilation, stent placement, over the scope clips, x-tacks and more. This lets fellows practice in a relaxed environment where there’s an opportunity to learn.

The fellow experience

Overall, you’ll receive state-of-the-art training in consultative gastroenterology and hepatology at a nationally recognized, integrated health system. You’ll hone excellent endoscopic skills and participate in meaningful quality improvement and population management projects.

Geisinger was one of the first hospital systems to adopt Epic, which will also give you access to data not available at most large academic centers. This is helpful in day-to-day care for patients but also for research. We also have a robust library and research department to help with literature collection and data retrieval from her. And biostatisticians are available to assist with statistics.
